Anything is possible, I think if the petition is reaffirmed its highly unlikey they will deny it the second time around. My petition was denied and reaffirmed, out next interview is next month.

Can a reaffirmed petition be denied again or after one denial, visa is issued.

Be advised the DOS can do what they want to and there is no legal channel to appeal. Do not give them a reason to deny. Be prepared and well documented. And if they ask a yes no question, answer Yes or No, don't elaborate. Don't offer what they don't ask for and be polite and respectful. Have documentation handy.

A reaffirmed petition is returned to the consulate for a new appointment and interview. IF something that was unknown/undisclosed is discovered at this time the petition can be redenied. It is highly improbable that this will happen but it could.

This will not be over quickly. You will not enjoy this.
