Hello, I have a question on filling out the I-130. It is ques D#2. It states" have you ever filed this petition for this or any other alien? When, where, and decsion? For me this is yes, for my ex.Do I put where we sent it? It was Nebraska, and when, is that the date it was received?

I think they mean the date when you sent the I-130 for your ex.

You answer the question, all of it. When, where, AND the decision. Read carefully and interpret literally. Works every time it's tried.
