I am submitting the AOS and have my DS-3025 which has my results as "Vaccine History Incomplete" then "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above). Do I need to go get the I-693 done or do I just submit the vaccination documentation worksheet with the AOS and cross my fingers that they have the medical exam info???? I have an appointment at 12 and would rather save the money.

It's completely up to you.

Send the DS-3025 and you might get an RFE for the vaccination transcription. The slows down your case by up to 2 months (usually not so long).

Get the I-693 and it might not be needed so you wasted the money, but you're less like to get an RFE.

In neither event are you going to be denied so the qn's are:

1. Do you want to spend the money when it might not be needed?

2. Are you okay with getting an RFE if it IS needed?

Personally my shots were up to date and I knew it but my form said "incomplete", I still got the transcription done because I didn't want an RFE. $50 well spent in my opinion but it's your decision. If you know (or think) you're missing shots then you should definitely get the transcription (and the shots you need).
