Hi,For a Manila submission, on DS-156 Section 2, Place of Issuance (passport)= Cayayan de Oro (on Mindanao), USEM instructions say "for applicants whose passports were isseud in Manila, please leave "Country" & "State/Province" fields blank in Item 2, should I fill in "country" & "state/Province" fields since my fiancee's passport was issued outside of Manila ? Thank you for all your wise advice so far

I think her Passport Issued in Manila so...Place of Issuance: City;DFA Consular Office Mnl. Country;Philippines State/province; Cagayan de Oro. God Bless Us

No, I told you that my GF's passport was not issued in Manila, why do you insist that it was issued in Manila.

Please read the question.

It was issued on Mindanao, 1 1/2 hours by plane south of Manila.

So should I fill in the "Country" and "State/Province" spaces in Item 2 of the DS-156 Form ?

05/02/2022 St Luke Medical appointment

05/23/2022 Sputum test wait 2 months to released result

07/22/2022 Sputum test passed same day Immunization

07/27/2022 Interview US Embassy Manila (Refused ask Embassy request DNA test)

08/31/2022 DNA test Kit Paid (Universal Genetics)

9/13/ 2022  Embassy took the sample both me after 3 days result was 99.9999 % Positive :)

9/29/2022    Passport with visa 

10/2/2022    CFO registered online only  

10/2/2022  Paid 220 for green card

10/19/2022 Flight to USA 

10/20/2022 Arrived in Alaska USA

State/Province- Mindanao

I guess that would be the right..

Cagayan de Oro is the provincial capital of the province of Misamis Oriental (Mindanao is not a province, but a huge island with many provinces)

So I would put

Place of Issuance : Cagayan de Oro

Country: Philippines

State/Province: Misamis Oriental

Dec 05, 2011: Received U.S. permanent resident status (AOS based on K-1 visa)

Dec 16, 2013: Removal of conditions on green card

Apr 28, 2015: Became a U.S. Citizen
