If you were married in the US, you need to report the marriage to the Philippine Embassy or Consulate that has jurisdiction over your City/State. The NSO will not accept a foreign document without going through the consular process.

The report of marriage is a Philippine Govt. document, not a USA Govt document (K1 is a USA visa). The Philippine government needs the report for records back in Manila. If you intend to update your passport at some time in the future to your married name, you will need to complete the document. Directions seem straight forward. Just go to the Philippine embassy website in Washington DC

http://www.philippineembassy-usa.org/

and find the consulate that serves your area. They have an outreach program where the embassy comes to cities in your area.
