ok so when i go see the civil surgeon i am basically just asking him to transfer my vaccination supplement sheet onto one of his I-693a sheets? i dont have to go through the whole medical, because his lovely nurse tried to insist that i needed it, to which i flat out said no (my medical is not a yr old yet) plus i have had all the nessecary vaccinations, just need them transfered......plus anyone from the UK who had there vaccination supplment sheet transfered over here by a civil surgeon, what did you do about the varicella shot, as its not available in the UK?

Woo Hoo....8/28/08 Recieved 10yr Greencard in mail, no approved email!! We are done!!!

You might want to print a few out and bring your own, just in case. Especially since the Civil Surgeon you are dealing with claims to not have much experience with it....

Also, Varicella is Chicken Pox, so if you've had Chicken Pox, you should be exempt.

My husband is from the UK, and that's how it worked for him at least.

Yep, all you have to do is have the civil surgeon fill out that one A4 sheet for you saying that you have had the MMR and TD vaccinations. Then ask them to put in in a sealed envelope for you. You dont need no medical and no more vaccinations and they shoudnt charge you too much to fill in the form. Mine cost me $25.

well ladybutterfly i feel i'm getting screwd over the cost, they wanna charge me $80 for an office vist, i explained what i needed but still $80 .....i have no choice as that is the nearest one to me and the other office i tried calling, the number is disonnected

$25 is VERY cheap....$80-$100 is what we were quoted as well for just the vaccination supplement. You're not getting ripped off. Just make sure to take the previous advice and bring your own I-693 vacc blank forms and also get an EXACT copy of whatever they are putting in the sealed envelope. Why? See the havoc that Faith has been through because the CS put the wrong documents in their sealed envelope:

yES the surgeon are just going to transfer your vaccine history i paid 40$ and he didn't put it in a sealed envelope but we were able to send it with the aos applications .My husband thought we sent the original vaccine supplement but we didn't hehe we have the original so were just bring it on the interview. The civil surgeons will try and tell you that you need to do a full medical, just to try and get money out of you. You do not need the full medical. If they refuse to do just the supplement, try another civil surgeon.

I called mine twice to make sure they would do just the supplement and they said it was fine. Then once I got down there, the nurse tried to hassle my husband into doing the whole exam anyway. Just be firm about it.

Once you get there, show them your DS-3025 and any other vaccination records or booklets you may have with you. From that, they will transfer the info from one sheet to another themselves. You just have to fill out some information about yourself and give a signature. And that's it! Should take about 15 minutes, tops. They'll give it to you in a sealed envelope, and you just make sure you don't open it.

The civil surgeon Claudeth saw also tried to force a full medical so I told them to look it up. Then they tried to force her to have another xray so I whipped out the one from the medical! After all was said and done they just filled out the form for us and charged me $45 or something like that (heck, I don't remember the exact amount now)
