My Fiancee received her P-4 and many pages are missing. All she received was the DS-156 and DS-157 with an appointment letter. She did not receive the K1 letter of instructions, GNI-2, DS-156K or any medical information at all. I know I can print these off the websight, but I guess my main concern is that the medical forms listed do not match the letter that states that all forms will be generated by the hospital. Will she need to print out all the medical forms listed in the P-4 package at Guz websight or just take her passport, photos and interview letter.

Please print themm off, use the ones on the website.

Contacting GUZ IV for replacement forms is almost impossible, then is another 2 weeks of waiting, should they actually respond to your email.

Sorry for the cr@ppy answer, but it IS a cr@ppy consulate.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
Ya know, you can find the answer to your question with the advanced search tool, when using a PC? Ditch the handphone, come back later on a PC, and try again.
