It is not required, but depends on other evidence that you have! If you can provide everybody here with a list of what you are sending we might be able to help you better.

3. Copy of Insurance policy listing X as the beneficiary.

iii. Other relevant documents

1. Copy of X & Y drivers’ license.

2. Copy of X & Y MVP Health Insurance card.

3. Copy of X & Y AT&T phone bill.

iv. Affidavit Letter of Relationship Evidence from L.

v. Affidavit Letter of Relationship Evidence from Mr.S.

vi. Affidavit Letter of Relationship Evidence from Mrs.S.

You have an excellent list of evidence. As the others have said, no need to notarize. I sent several affidavits and they were not notarized. I added the following per advice from my lawyer-friend who helped guide me:

"Signed under penalty of perjury"

Not sure if that helps, but that's what I had.
