Just wonderin how and where did you get a copy of health insurance and 401k stating your spouse as beneficiary. My husband asked their HRD admin to provide him a copy of those but the admin said that they dont have such thing instead she suggested to go to company website and print the profile information were in the spouse was the one who will contact in case of emergency. Is this suffix as a proof?

P.S

Im not covered with his health insurance because we can`t afford to pay like 350$ a month.

I think printing out from the insurance company's website will do the trick...

I wouldn't worry about health insurance. Are you planning on including a cover letter? If yes. you can consider writing the reason there...

My company does the same thing. I'm still putting the package together, but in case I want to throw them in I've captured screen shots from the internal corporate HR site showing the beneficiaries and dependents covered by my insurance. I also have an email from HR that states that the company doesn't produce documents showing coverage or beneficiary status, and suggests I capture screens for this purpose. I've also gone out to the insurance company's website and downloaded the latest statement, which shows claims for all of us, as well as their covered status.

We had medical cards with our names on it for proof of joint medical insurance. I also included some bills from insurance company showing my husband as patient and the same address.

For 401k and other benefits where my husband is named as the beneficiary, I just printed the elections confirmation page from when I signed up (or renewed) them.
