Depends on the consulate (which your profile does not indicate) - but normally, you leave your passport with the consulate and it is sent to you by messenger to you once it is ready. How long it takes, again, depends on the consulate as well as how busy they are, etc.

When the K1 visa is approved will my fiance be able to have it that day or do we have to wait? ( a week)

The consulate in Ciudad Juarez!

CJ will usually NOT tell you if your visa has been approved or denied on the day of the interview... they will say they'll let you know 7-10 days.
