CANT FIND THE FORM FOR AFFIDAVIT OF IMMIGRANT STATUS!!!

WELL I AM A "PERMANENT RESIDENT". I LIVE IN FLORIDA.

MY HOMETOWN IS BILIRAN LEYTE PHILIPPINES. I JUST WANTED TO GET EVERYTHING DONE BEFORE MY PASSPORT EXPIRED. I AM GOING HOME THIS YEAR AND I WANTED TO CHANGE MY LAST NAME ON MY PASSPORT. I GOT HERE AS A K-1 VISA.

I DONT KNOW WHERE TO FIND THIS " AFFIDAVIT OF IMMIGRANT STATUS " AT ALL :crying:

There's a copy of the affidavit on the Philippine consulate's website in Vancouver:

I don't know why they asked for it at the consulate in Washington D.C. and didn't bother to provide a link to it. Other Philippine consulates in the US apparently don't require it. Maybe you should email them and see if it's still required.

12/15/2009 - K1 Visa Interview - APPROVED!

12/29/2009 - Married in Oakland, CA!

08/18/2010 - AOS Interview - APPROVED!

05/01/2013 - Removal of Conditions - APPROVED!
