DS 11-application for US passport

hello to all members..and for those who have been with the CRBA And us passport with your kids please share me your information about with this matter..please!

i was confused with the forms i received from fedex/air21.. :help:

forms:

DS-2029-application for consular report of birth abroad

DS-5057- affidavit of parentage and physical presence in the US

DS-3053- statement of consent

DS-11- application of US passport

my question, they sent me a two DS-11 form:

the one form says

U.S. Department of State

APPLICATION FOR A U.S. PASSPORT

PLEASE DETACH AND RETAIN THIS INSTRUCTION SHEET FOR YOUR RECORDS.

I applied: Place:

Date:

while the other DS11 form:

APLICATION FOR US PASSPORT

US passport Book Or US passport Card

Name

First & MIddle

i was so confused about this :(:unsure:

do i have to fil up the two Defferent DS-11 forms?..please gave me some info for those who have been with the process....thank u! and God Bless US :)

It's fairly simple, really. The form that says detach and fill out the place and date is for you to keep - kind of a proof for yourself when you applied for the US passport.

The second form is the real form that needs to be submitted if you want to get the US passport.

Not sure if I really understand the question, we just downloaded a fillable form from http://www.state.gov/documents/organization/79955.pdf that can be very neatly filled out on your computer and carefully read the printing instructions. Have to bring this form with your photos and ID to one of the 9,000 agents and don't forget to bring money. Our agent checked our form out line by line.

http://travel.state.gov/passport/forms/ds11/ds11_842.html has all the instructions, really hate to admit this, but always had problems with the more recent government forms, seems like they don't know how to phrase a simple question and may have to read the instructions 2 or 3 times until it finally sinks in, in what they want. Yes, I am a college graduate with 22 US patents under my belt, so not exactly an idiot, but close.

Apparently I am not the only one, see hundreds of web sites that offer services to fill out the DS-11 and other government forms, but you have to pay for this service just like millions pay a fortune to have their income tax forms filled out. Can only wonder if the government is getting a kickback from all these places by writing nebulous questions and instructions, like sentences with two or more negative statements in them.
