Hey everyone.

Just a quick question. On DS-156 when it asks about the passport's issuing city, did you put anything at all?

Since there's no mention about the issuing city in Daria's passport I'm not sure whether to leave it blank or not.

Also, for State/Province, did you put ".... oblast" or left it blank as well?

Another thing: national ID#. There's a number in her international passport, "personal#" is what it's called.

Should I use that?

For the issuing city, you can just put the city she received the passport in, probably the same city she lives.

That's what I was going to do, was just making sure.

Any thoughts on the ID# line?

Unless you're sure that they give out national ID's in Ukraine (I don't think they do, but I can't say for sure), then just enter "NONE."

I assume national ID is akin to our SS#...

I don't need to finish the form today, so I'll wait what others that went through Kiev say. Just want to make sure the paperwork is complete and correct.

There should be an ID# in her "propyska" (ie. internal passport)... Probably the same as her personal # in her international passport

I would have to check with the wife for sure, but I am fairly confident it was...

Yeah, I just talked to Daria, she said there is a number for sure. She'll find it later on.

I'll post the info for the future reference about where exactly to find it.

we used "Donetsk" city "Donetsk" for state/province (it is also the name of the Oblast)

We used the number from the internal passport

I'm just going to put "Karkov Oblast" and the number she found (it's on a separate piece of paper too that is given to them)

The internal passport lists the MP#. This is printed on the internal passport itself. Yes they do have a separate paper that also lists a number as well to compliment the passport. I guess for security. Not sure if someone can clarify what the correct number would be. I assume that the MP# is how the internal passport is linked to the system. Key words...Assume. I also need some clarification on this as well.
