Guys is the form G-325A ia composed of 4 pages or only 1?????

I filled it from the USCIS website and printed. It was only one page. Can anybody clarify to me this please?

Hope everything goes smooth :) It can be frustrating, My stuff is sitting in it's package just waiting for the civil surgeon paper... Can't wait to mail it monday or tuesday. Inshallah

My check was cashed yesterday. only 4 days after receiving my package, which is faster than what I imagined. Alhamdulellah(all praise to god).

[April 27, 2010]: Received RFE, they lost medical form

[April 19, 2010]: Received biometrics appointment

[April 19, 2010]: Received NOAs for I-130

[April 16, 2010]: Received NOAs for I-485, I-765, I-131.

[April 12, 2010]: Checks cashed. Case established on USCIS website. [Day 9]

[April 12, 2010]: Received text messages and eight emails from USCIS with MCS number [Day 9]

[April 5, 2010]: AOS package received and signed by USCIS [Day 2]
