My husband and I almost have our AOS packet ready to go, but we're stuck on something -- the medical exam.

My husband came into the country on a K-1, so he's already had his medical while he was in England. I saw the instructions say to include the Vaccination Suppliment, but my husband *swears* that it was in that sealed enveolpe that was taken off of him at immigration.

What do we do?

an I-693a document must be filled out by a civil surgeon...this doc simply transcribes your vaccinations to another form signed by the civil surgeon.....if your medical exam is less than 1 yr old, you do not need to do it again......

I'm so sorry, but I'm still confused -- do we have to pay another $200 and find a civil surgeon in the US to fill out that vaccination suppliment or, since his last exam was just in May, do we not have to do it?

Do you know if there's some way to have the civil surgeon who looked at him in May forward the results to the Filing Center here -- or to us directly so we can send them in with the package?

Hey!! You don't have to do the medical exam all over again, that is for sure!! What you need to do is get the I-693A supplement form filled out by a civil surgeon and to do this he needs his DS-3025 form. He should have received a copy of this form from the doctor when he got his medical. If he did not receive it then he will have to call the doctors office and ask them to either fax it to him or mail it. My fiance had his doctor fax it and he got it within minutes. This form lists all the information the civil surgeon will need to see to give you a supplement form! It should only cost around $25 or so. Good Luck and if you need more help just ask!! :D

Do what my husband and I did. All we had was his original dr. documents from Ukraine. We found a dr. in the area from calling the INS office who was able to read the Russian to see what shots he had and when. It took 30 mins of waiting and 5 bucks for the form (he didn't need any shots). Just call a C.S. okayed by the INS and ask if they will be able to do the form for you using his medical history from his PCP. If they want to charge you a lot of money or mention doing the whole exam over again, hang up and try someone else!

(P.S. Your husband was right about handing it over when he entered the country. They took my husbands as well, which they are suppose to. Your husband just didn't get the other form he needed when he did his medical exam.)

I feel so stupid for asking, but do you know where I can get a list / find a site to locate a CS in the US in/near my area?

another thing...

you should really call around to different civil surgeons to find out how much they will charge you for filling out the form. we found the price varied wildly from one office to the next, and the best we could do was about $35.

My husban took the medical exam in Morocco to come to the United States on the I-129.. His I-130 was approved and wanted to try and get his residence through a piliot program in Dallas.. My question is he does not have copies of his medical exam. A
