So you didn't send an DS 3025 from your medical or a I-693 from a civil surgeon with your AOS? Seems strange that you've received an interview letter without an RFE for this information.

If you had a medical less than 12 months ago then you won't need another one. Take the DS 3025 with you to the interview.

I didnt send any i still have the vacination report with me and it is valid until march 8th 2010 and my interview is in November 2009 , i think the embassy enclosed my medical in the envelope they sent me the one i handed to the immigration officer at the POE , and i assume i dont need a new one , so you think i just need to take the form i have for the vacinations???

what about u MonkeyJuice , did u have a medical back home ? and what did u do ?

I came to US on a K1 visa & my medical report was also in the envelope that i passed to the CO at POE.

I also included the I-693 form with the AOS application which included all the the vaccinations I got in UK & also the chicken pox that I received in USA. The I-693 was completed by the Dr Arnold in Texas.

If you have all the vaccinations required I would just take the DS 3025 form that you received at your medical.

I haven't read anything on VJ about anyone NOT sending in their vaccination records with their AOS application. Maybe some one on here knows from experience.

hello,

I just got done with my AOS interview last week...yeah, u need to see a civil surgeon to have med. examination and additional vaccine , civil surgeon will give you a sealed envelope which is the I-693 form(with sign) that u will be needed for the day of your interview. during my interview the officer dont ask any papers except the sealed envelope from the surgeon..i ask the officer if she need some more papers she said no, since they got it already...but, i advice u to bring more extra papers that u think u will be needed during your interview...i was lucky coz the officer i had she was very nice....Good Luck to u!

What if I am a K nonimmigrant visa holder and

already had a medical exam overseas?

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3

spouse of a U.S. citizen, and

C. You received a medical examination prior to

admission, then-

a. You are not required to have another medical

examination as long as your Form I-485,

Application to Register Permanent Residence or

Adjust Status, is filed within one year of your

overseas medical examination.

b. You will, however, be required to complete Part

1, Information about you, and submit the

vaccination section of Part 2 with your

adjustment of status application. A designated

civil surgeon must complete the vaccination

section and Part 5, the Civil Surgeon's

Certification.

I had problem understanding this form also and had called the USCIS 5 times in one day to confirm this. I got different answers but i still did go to a civil surgeon with my DS 3025 together with this instruction from the form. Note : set an appointment and explain to them your situation that you you had your exams and vaccinations before and will just let the civil surgeon sign the Part 5 of the I-693.

Good luck..

Do you received tons of things that you needed to bring for the interview?
