A month ago I filled out the G-325A. It had 5 pages, four you could fill out and were the exact same sheet. It also says on the USCIS webpage that there are four sheets. This sheet had an expire date of 5/31/09

I went there today to look and the USCIS webpage still says four sheets but when downloaded there are only two sheets, with only one copy of the information and the other being instructions. The document has an updated expire date of 6/30/11.

Looks like they only need one sheet now. Anyone else know about this. Also, can we get the G-325A download on this site corrected fast so no one has any problems?

There are four pages because they go to four different offices. Sign & send four pages what ever effort that takes.

The form was updated some time in July (to the single sheet). The instructions on the site still need to be changed to reflect this single sheet. Currently the direction is to include only the one sheet (as opposed to 4 from the previous version).

Some folks are not taking any chances until this is positively clear and are submitting 4 copies of the new form - the thinking is; "If they don't need them, they can throw them away."

Edited to add:

There is also a date field issue with this new form (doesn't allow proper number of digits) which will have to be addressed by USCIS as well. The workaround is to simply hand-write this field in once the form is printed.
