We are in the process of putting together our AOS packet and I have a question about the I-693 form. I've been searhing around on this forum and have not been able to find my answers. My wife came to the US on a K1 visa 2 months ago. As part of her K1 visa she had a medical examination in her country about 3 months ago. In the I-693 instructions it says:

"What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3

spouse of a U.S. citizen, and

You received a medical examination prior to admission, then-

You are not required to have another medical examination as long as your Form I-485,

Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination.

You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification."

1. So I am assuming that I do not need to include the I-693 in my AOS packet. Is this true?

2. Do I need a vaccination supplement?

3. Also how will USCIS know that the examination has been completed?

4. Is the exam on record with them somewhere?

1) if it has been less than one year a new exam is not needed

2) did the panel physician fill out the DS-3025 at the exam? Did she provide vaccination history at time of visa medical?

3) It was in the brown envelope that was provided at the POE... they will call the file up from storage

Per I-485 instructions, it clearlty states that a medical report (I-693) is REQUIRED to be submitted for AOS applicants.

Per I-693 instructions, if you had a medical within 1 year of applying, you don't have to have a new exam and the whole form filled out, just need the vaccinations section and doctor's signature.

Note: some people have gotten approved by only sending in a copy of the DS-3025/2053 they got abroad, but not all. If it's not accepted, you will get an RFE.

Do I need to make an appointment with one the USCIS civil surgeons to only have the vaccinations section completed?

or can I use the below dr from captain ewoks post?

To have your vaccination information transcribed to Form I-693, the following Civil Surgeon will do it by phone and fax for a nominal fee ($10 at time of writing):

Dr. L. E. Arnold

4534 Westgate Boulevard, Suite 106

Austin, TX 78745

Tel: (512) 444-3131

Fax: (512) 447-4699

I believe the price is now $30, also heard of some people having problems because he's not in their state (you would need to search for that one). I found a civil surgeon close here in my state that charges $25, but I do live in a major metro area of state.

So correct me if im wrong, I need to make an appointment with a local USCIS civil surgeon and have only have the I693 vaccination section filled out. I'm going to check around to see if any dr.'s can do it over the phone like dr arnold listed above.

impossible to do it by phone... Dr Arnold does it via mail
