I did not realize until today that my wife needed a medical exam for the I-693a. We need to file for her AOS on or before this Friday 7/31/09. I was under the impression that the DS-2053 Vaccination Worksheet was enough. My question is should we file Friday then submit the I-693a after since we probably cannot get it filled out before Friday? Or should we wait for the I-693a to be filled out and apply late for the AOS? Thanks in advance.

She already had an exam in the Philippines .. I take iit.

Fill out the form and get to a Cival Surgeon only to update her shots she did not get.

They will give you the form and whatever in a sealed envelope to send in with your AOS AP and EAD forms

They well not exam her since she has already had her exam.

Remember 6 photos 2 each .. for AOS ... AP .. and EAD file them all its all part of the 1010 dollars and it is best

to do them all at one time.

FYI-There is no deadline to apply for AOS. Ideally, it's best to apply before the I-94 expires, so you are not "out of status" but it's not a big deal, people have waited a couple of years and were approved.

If she had a medical exam within 1 year of applying for AOS, then technically she doesn't have to have one. You still need to send in form I-693 (not I-693a, that form doesn't exist anymore), but only the vaccinations section needs to be completed by the doctor. They should transfer the info from DS-2053 to I-693. The problem is that many civil surgeon (that's what they call the USCIS-doctors that do medicals for them) try to get the applicants to do a full exam, even though they don't need one. Most CS's won't fill out just the vaccinations part, they try to convince you that you NEED a full exam. They want to charge you you more, so they lie their asses off. It should cost under $50 to complete the vaccinations section and sign the form, but doctors charge as much as $400-500 for a full exam.

Search for a CS in your area that will agree to transfer the vaccinations and sign your form without a full exam. If you can't find one, you have 2 options:

1. do a full exam and pay for it

2. contact Dr. Arnold in Texas. He's a CS who many people here have used. He will transcribe the form through the mail. He charges $30 (I think) for this service. Do a search for his info, if needed.

Thank you soo much for the info. I believe I found a doctor that will do it for $80.00, not the full exam of course. If that doesnt work out I will find Dr Arnold.

She will be required to undergo TB skin test (TST). If she's positive (which most Filipina are), the doctor will require her a chest X-ray.

If it were me I would submit the DS-3025 vaccination history with the I-485 and not bother with the CS. YMMV

Thank you. I will go to the medical clinic tomorrow and they cannot provide me with the needed info on the I-693 from the DS-3025 that I have I will file without it and wait for an RFE if it happens. Thanks for your input.
