yippee ! every single civil surgeon in the area REFUSES to transfer the ds-2035 data.They require the FULL medical and FEE!!!! of fkn course

prices ranged from $110 - $485. All were very rud and all called me a liar when i explained they only need to transfer the vaccine data sinc the medical was only 4 months ago

of course the only cheap one is booked thorugh october! #######

Dr LE Arnold will do it via the mail for $30 or just send the DS-3025 with the application.... it worked for us

FWIW.. the I-693A is an obsolete form since May 2008. It is now Part II Item 5 of the redesigned I-693

so do i not need to do anything then? do i still need the civil surgeon to transfer the ds-3025 or not?

many have been successful including the DS-3025 only (my wife included).... If it were me (and it was)... I would submit the DS-3025 and wait for an RFE or interview before I spent any money I might not have to. The flip side is that with Dr Arnold it is $30 insurance.... Your call....

The answer is in the AOS sticky:

Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Determining if your vaccination's were completed is the hard part for most.

Ensure your beneficiary makes and brings with them to the States a copy of the DS-3025 (vaccination form)

If the government is going to force me to exercise my "right" to health care, then they better start requiring people to exercise their Right to Bear Arms. - "Where's my public option rifle?"

We used Dr. Arnold. He transcribed info from DS-3025. Everything went fine.

5-15-2002 Met, by chance, while I traveled on business

3-15-2005 I-129F
9-18-2005 Visa in hand
11-23-2005 She arrives in USA
1-18-2006 She returns to Russia, engaged but not married

11-10-2006 We got married!

2-12-2007 I-130 sent by Express mail to NSC
2-26-2007 I-129F sent by Express mail to Chicago lock box
6-25-2007 Both NOA2s in hand; notice date 6-15-2007
9-17-2007 K3 visa in hand
11-12-2007 POE Atlanta

8-14-2008 AOS packet sent
9-13-2008 biometrics
1-30-2009 AOS interview
2-12-2009 10-yr Green Card arrives in mail

2-11-2014 US Citizenship ceremony
