Medical exam (I-693) is required to be attached to each I-485.

I-130, follow: http://www.visajourney.com/forums/index.ph...gration-parents

I-485, Is same as for a spouse only things like Marriage cert, is not needed.

Definitely get the medical part done and send the I-693 along with the AOS package.

Most of the info related to required documents you'll find on the I-130 and I-485 on the instruction section of both the forms. These forms are easy to fill and in most part self explanatory. I-693 is initial evidence and should be submitted for both your parents when you are submitting the I-130 & I-485. The FBI clearance and name check is not your problem. Once you submit the documents, they'll start the background checks on their own. The medical exam and completion of forms should not take more than 3-4 days. You'll receive the results in a sealed envelope and that should go into the envelope with the rest of the documents for AOS. since you are applying for both the parents, you should be looking at somwhere between $350 to $500 expense on their medical exam.
