do we used her maiden name or my name for the AOS forms. We just got married on june 27th. we need to send out papers and dont know which name to use.

what other evidence should we send out with the forms?

We used our married name. You have to send a certified copy of your marriage certificate. The list of items is on the Guides section. It is pretty straight forward. Good Luck!

If she is taking your last name, then use your last name. If she is keeping her last name, use her last name. The green card will be issued in whatever name you list on the form. If she is changing her name to yours, the marriage certificate is considered a legal document by which to do so.
