Hi. My dad is bringing his family over and we are currently filling out the i-846 form. It's a family of 5, does each family member need to fill out an i-864 or can he list them all on one i-864.

For example.

Part 2 Principal Immigrant = Dad's brother

Part 3

9. filing out family members name..

print a new i-864

Part 2: Principal immigrant = Dad's brother wife

Part 3:

9. filing out family members name..

and etc......

Is this how to do it? or we can just use one form?

Is now a US Citizen immigration completed Jan 12, 2012.

This has nothing on point about the I-864 Affidavit of Support. You need one I-864 for each case. So if your dad is the primary beneficiary and the rest of his family are derivative beneficiaries, then only one I-1864 is needed as there was only one I-130 petition (i.e., one case). It is only when different petitions are file for two different people, (i.e., a petition for a wife and another petition for a child), then two I-864s would be required for the different cases.
