Hi all.

I'm putting together my I-485 but I'm a little confused. Lots of the forms requested on the I-485 instructions were already handed over at POE. I have copies of them but why do they need the same forms again? There is also the form they request for the Medical report but i also handed that over. Anyone like to help me out here :)

Which forms are you confused about? For instance the G-325 needs to be filled out again. The medical doesn't need to be repeated unless it was done over 6 months or a year ago I think. You just need to send in the vaccination supplement. There is an AOS forum farther down the page that is more appropriate for I-485 questions, FYI

Hi all.

I'm putting together my I-485 but I'm a little confused. Lots of the forms requested on the I-485 instructions were already handed over at POE. I have copies of them but why do they need the same forms again? There is also the form they request for the Medical report but i also handed that over. Anyone like to help me out here :)

You need all new forms even though you have already given some of this information before.

1. I-485

2. Two (2) Passport style photos of non-us citizen (I believe the rule is no more then 60 days old)

3. Copy of non-us citizens birth certificate

4. copy of non-us citizens passport biographical page and k-1 visa page

5. form G-325a for non-us citizen (all 4 pages) (you filled this out during the k-1 visa part but you need to do it again---you should now have a AA# if you didnt have one before and might even have a SS# that can be applied to this form now). Make sure for recent work that your ''present'' job is ''umemployed'' because technically you are not allowed to work until you receive your EAD :)

6. I-864 affidavit with copies of supporting documents

a. new employment letter from employer stating yearly income, current standings with company (prospects with company), full time/part time, when began job, title of job

b. copy of 3 federal tax returns including copies of W-2's (08, 07, and 06)

c. copy of six (6) recent pay stubs

d. copy of US citizens birth certificate

7. photocopy of i-797 notice of action (i-129 NOA2)

8. photocopy of form I-94 arrival/departure record (copy front and back)

9. Medical form from civil surgeon (we didnt have this and he had his medical done in the UK so we just sent the DS-3025 immunization record from knightbridge doctor)

10. certified copy of marriage certificate (this is NOT a copy...this is a CERTIFIED copy from your local judges office)

sorry i didnt really answer your medical question....

since you handed it over you are going to have to re-request the form (call the doctor and ask for a immunization record of all shots) make sure a nurse/doctor signs this

(thats my personal opinion on what to do to rectify your situation) there of course might be a better way to go about it

I actually have a copy of my medical and vaccinations signed by the doctor. Just remembered I was given a copy for my own records. Will that do?

Hi all.

It's a new application and like starting over. It's going to a new place and new people. Your packet from POE isn't in their hands when they open you new AOS application. I can't think of any form that would be the same. The G25A is different. You are married and maybe have a new last name (so also a former name). You have a spouse name and a new address and are unemployed.

There is also the form they request for the Medical report but i also handed that over. Anyone like to help me out here :)

You K1 medical info (from POE package) is supposed to hook up with your application before adjudication. They send for it so you don't have to submit the doctor report. They want it in a sealed package. If you had all your shots abroad, then you can submit a copy of the shot record. If you still need shots in the US, then you submit an I-693 with the immunization part only filled out and signed/sealed by a Civil Surgeon.
