Visawaiver AOS filers walkthrough and questions

Timeline

I'm assuming the instructions here are valid for those petitioning for AOS after marrying on the visa waiver program?

Does anyone know where one may find some examples of already filled out forms? There are some areas that are not so clear in the instructions and I also want to be sure I put n/a or none where I need to. Currently, I'm putting n/a in every single box that I have nothing in.

Do you all put the married name, or maiden name on the forms? Her passport and I-94 has her maiden name so I want to avoid any potential delays if that could be a problem.

We plan on filing everything around the middle of this month.

Timeline:

03/03/2009 - Arrival in the US!

05/03/2009 - Married!

05/28/2009 - Filed I-130, I-485, I-765!

06/04/2009 - Checks cleared!

06/08/2009 - I-765/I-130 NOA Received!

06/10/2009 - I-485 NOA Received!

06/11/2009 - Biometrics appointment letter, scheduled 06/23/2009

06/17/2009 - I-765/I-130 Touched!

06/19/2009 - Attempted early biometrics... they said nope!

06/23/2009 - Biometrics completed.

07/07/2009 - EAD card production ordered.

07/13/2009 - EAD card production ordered, again.

07/14/2009 - I-765 - Approval notice sent.

07/15/2009 - EAD card received!

07/20/2009 - SS Card applied for.

08/05/2009 - Received Interview letter - Scheduled 9/14!

09/14/2009 - Rescheduled because we moved to Dallas

10/14/2009 - Interview - approved!

10/23/2009 - GC Received.

Check out the page Karin und Otto mentioned - unlike K1/K3 filers, you do need an I-130 and the medical ( I-693). There are example forms if you look under the search bar in the blue area at the top of each page. If your wife wants to use her married name, fill in the forms with that, if she wants to keep her maiden name, use that. You are supplying the marriage cert, so there will be no confusion, it's hardly a rare issue. If she wants to change her name, it's much easier to just do it now rather than try to change later, since the GC will have the name you fill out the forms with. Check out the specific instructions on the USCIS website for each form too, just to make sure you have the most up-to-date info. Good luck, we also adjusted from VWP.

Is the I-693 required at the initial AOS filiing?

I have another question but not sure how many can answer this one. Her first name is in Latvian on her passport and is spelled a bit different in English. The latvian ž is zh in English (Snežana and Snezhana respectively). Currently, our marriage license has her named spelled with only a z (Snezana). Her birth certificate is in the process of being translated at the state university and her first name will have the zh. Will this be a problem? If so, should we fill out all forms using the z instead of zh so it will match her passport, but without the accent? I'm thinking the birth certificate translation will be enough to avoid any confusion...at least I'm hoping so, she wants to use the zh version.
