Applying for AOS - Medical records

Timeline

Hi everyone. Just wondering if I could get some help. We are applying for AOS for my wife. I read that medical records are not required if she has had a medical exam done for the K-1 Visa. However, the vaccination supplement is required. Now, when she had her medical exam done she did not have to get any vaccinations. So I was wondering what do we need to submit with the AOS application? Immigration kept all of the documents that we had including the medical reports.

Any help would be greatly appreciated. Thanks.

Timeline

You don't need another physical. You only need the civil surgeon to fill in the vaccanation portion of the form. You need to give the civil surgeon proof of vaccanations. The center for disease control (CDC) website has the vaccations you need. Any females under 26 years (I think) need the HPV shot also.

My wife needed vaccinations and she got it at an aproved hospital - the one aproved to do her medical exam. If you wait over 1 year you will need a new exam by a civil surgeon here. If not, and vaccinations are current and valid, you just need:

I-693 Part 1 Information, Part 2 Vaccination (supplemental),

Designated Civil Surgeon section,

Part 5 Civil Surgeon's Certification

But I don't know if they keep the medical exam record at PoE and retain them for my AOS, or if my wife didn't get them returned to her somewhere along the way...

From the guide:

"If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required "

2-6-07 NSC received USPS mail, NSC then to CSC

2-15-07 NOA1 -file received

2-16-07 check cashed

2-23-07 touched

5-4-07 NOA2 approval -email

5-13-07 sent cancellation request letter

6-7-07 we're going to retry with a K-3

8-6-07 married in Thailand (dual language, dual representation prenuptial)

8-7-07 sent K3 from Bangkok

9-10-07 I-130 NOA1, (received at CSC 8-9-07)

10-9-07 sent I-129F to CSC

11-1-07 touched I-130

requested consular processing I-130 (http://www.uscis.gov/files/pressrelease/PN_i-129f.pdf)

9-13-07 I-129F for Spouse arrived CSC via USPS return rcpt. requested

4-1-08 NOA2 for K3 (I-134 supposed to be processed but processed I-129F instead)

7-11-08 interview Bangkok, passed.

7-16-08 POE arrival, 2 hours in Seattle Customs.

AOS I-486 sent 4-4-09

AOS NOA1 4-13-09 for all; I-485, I-131, I765

RFE 4-27-09 Thai official document in lieu of original Birth Certificate not sufficient???

Infopass appointment 5-26-09 at USCIS. Officer thought our doc was valid and doesn't know why the RFE.

7-28-09 EAD and AP sent

Social Security card 8-4-09

interview 9-10-09

10 year green card expires 9-17-19, Permanent Resident Card.

Resident since 9-10-09.

Yep same thing happened to my husband...he handed all his documents over at POE. So after calling bribing (that is the only way to get things done over there lol) they gave it to a family member that had a copy of his passport picture and k-1 visa that we scanned to them. They finally gave it to them and they scanned it back to us. We then called Dr. Arnold and sent his information, they transcribe it for $30.00 etc. Any questions feel free to PM me.

January 15, 2009--Apply for SSN Number

Jan. 23, 2009---Married

March 2 2008----Received SSN card

March 14, 2009---Got Driver's License

AOS TIMELINE

Mar. 30, 2009: AOS/EAD/AP filled

April 7, 2009 - EAD,AP and AOS NOA notice date

April 10, 2009 - received NOA1 hardcopy for EAD,AP and AOS

April 13, 2009- received Biometrics appt scheduled April 25, 2009

April 14, 2009-Biometrics walk-in done!

April 14, 2009-Called USCIS for AP to be expedited.

April 16, 2009-AP approved YES!!

April 24, 2009-EAD Card Received!!
