Has anyone used Dr Arnold in the last couple of months and how much did he charge?

What did he need in order to complete the form?

it is $30 now. what i did was to print-out I-693 form pages 1,3 and 5 and signed the ist page. he will need your vaccination records and transcribe it to I-693 and they will mail it back to you. be sure that you will call them first before sending any documents.

Me and my hubby chose Dr. Arnold's office to do my I-693 last March 9, 2009.

He charges a $30 fee for transcribing your vaccination worksheet into an I-693.

Just leave a voice message to his office and his secretary will get back to you the soonest time.

The secretary will give out the needed requirements. Try asking about the needed vaccines in case it is incomplete.

If you think you completed the series of the required vaccines, don't bother asking.

They would usually ask for your DS-3025 (VACCINATION WORKSHEET), $30 check payable to Dr. Arnold,

2 printed copies of Part 1 of I-693 (signed and dated) which you can download at the USCIS website and on a separate sheet of paper

containing some personal info like your name, address, date of birth, place of birth, US SSN, phone number, country of birth and your A #.

Just recently got my I-693 today from Dr. Arnold's office via USPS priority mail.

Got the I-693 in the sealed envelope from Dr. Arnold last Saturday.

From the first time we called, it took about 2 weeks.

From the day we mailed him the documents needed, it took 9 days.

We hope we don't get a RFE on the TB test though...

wow, $30 now! when i send my stuff there at the end of 2007 it was just $10! but after that a lot of people from vj chose him to get their vaccinations transcribed, go figure!
