For the biographic information, or G-325a forms. I had to attach a separate form to include all my work information. Do I have to put, one attachment behind each of the 4 forms or am I allowed to just put one attachment from at the end of the 4 pages?

That's a good observation - most folks will simply include a single sheet (if needed) - attached to the back of the entire (all four pages) form. (Thats what we did.)

Looks like the 4 pages go to different places and if so I'd put the attachment on each individual sheet.

I have the same thought as Haole, so I put an attachment for each sheet. I had no problem.
