Also.. My wife adopted my last name, so.. does she need to file any paperwork with USCIS about the name changed?

Yes, the USC needs to fill in the G-325A and supply the photos it asks for. She can put her maiden name where it asks for previous/other names used and put the married name for her current name.

Removing conditions:

10/9/10 - Package sent to Vermont

10/13/10 - NOA1 date, received 10/18

I beg to differ: the US citizen spouse does not need to fill out a G-325A and supply accompanying photos - that is only necessary when filing the I-130. Jon & Lindsey, you filed I-130 and I-485 concurrently, so you included G-325A and photos for both spouses. The OP is adjusting from a K-1 visa, therefore only filing an I-485, which is the alien's application. So only the foreign spouse needs to file a G-325A and pictures along with their I-485.

I understand that your wife is the US citizen - she does not need to notify the USCIS of her name change, she is not immigrating to the US  She only needs to take care of her other documents: passport, SSN, driver's license, health insurance, etc etc.

Good luck!

In the form G-325A in the part about your parents residence, is this the last residence or should I state that they are currently dead?

I put down "N/A (deceased)" for my dad's residence.
