hi to all...i need some help about all other names used ....im not married before but i change my first name do i need to put my old name in that category or no need na...we are just starting to gather data's i hope someone will help us...

u know thats a great question and im not sure of the answer so this is just a guess on my part .........if u urself changed ur name and it was not changed as an infant because of adoption it means i think anyways that at some point u used ur other name legally in school or for bill so yes i believe that u do have to put it down when explanation that u had it legally changed to what it is now :whistle: then again i could be all wrong but thats what i would do

If you changed your name, you first used another, so list the other name you used.

Read carefully, interpret literally and answer accurately. Keep those simple ideas in mind as you fill out the forms.
