I would think that it means the name and address of where you'll be living. If Andrew lives with his Mom and that's where you'll be living, then that's what you put down. If Andrew is on his own, it doesn't matter who's co-sponsoring you, you put down that you'll be living with Andrew and his address. Unless you will be living with Andrew's Mom and not Andrew?

It asks for the address where you'll be staying - put the address. #25 asks for a contact name and telephone - put your fiancé.
