Yes, this is exactly what I did. I took her two forms, she signed them. I got all the information from her and made copies and then when I got home I filled out the forms to send in. I was only there a few days when we did this and I saw no reason to spend much of our viasit filling in government forms. I had her sign two forms in case I made a mistake.

Yes, this is exactly what I did. I took her two forms, she signed them. I got all the information from her and made copies and then when I got home I filled out the forms to send in. I was only there a few days when we did this and I saw no reason to spend much of our viasit filling in government forms. I had her sign two forms in case I made a mistake.

A word of caution if you are thinking of copying those forms....if you look at the lower left corner there are 4 separate pages. In fact, this is the only difference between all 4 pages. Chances are nobody will be scrutinizing them that well, but I was told that the form could be rejected if you submit 4 copies of page 1 or 2, etc. It's easier to fill out the forms on a computer and she will know what goes in those blanks and how it should appear (names, address, citys, etc). If she is going to sign them anyway, just have her fill them out too. She will have to gather all the info anyway and somebody still has to fill out the form.
