Hi everyone!!!! I just want to ask if someone knew already that USCIS changed the vaccination requirements for AOS to Legal Permanent Resident. I was about to set an appointment to an accredited surgeon of USCIS, until they told us that I should take a TB, HPV (Human Papillomavirus) shots/vaccine before setting an appointment to the surgeon for my I-693 form, I asked why? She said that's the new policy of USCIS of having the I-693 which is effective just this month. And that surprised me. So, what I did I searched in USCIS site to make sure.

Here is what I found out.

USCIS Changes Vaccination Requirements To Adjust Status To Legal Permanent Resident

Washington – U.S. Citizenship and Immigration Services (USCIS) announced today a revised list of vaccines required for applicants seeking to adjust status to become legal permanent residents. This revision follows guidance from the Department of Health and Human Services, Centers for Disease Control and Prevention (CDC).

CDC’s revised Technical Instructions to Civil Surgeons for Vaccination Requirements require the following age-appropriate additional vaccinations to adjust status to legal permanent resident:

Rotavirus

Hepatitis A

Meningococcal

Human papillomavirus

Zoster

The requirements for these new vaccines went into effect on July 1, 2008, however CDC approved a 30-day grace period for any medical exam conducted before August 1, 2008. At that time the new vaccinations, if appropriate, must be administered in order for USCIS to approve the applicant for adjustment of status.

USCIS has revised the Report of Medical Examination and Vaccination Record (Form I-693) to include these new vaccination requirements. The June 5, 2008 edition of Form I-693 must be used for any medical examination completed on or after August 1, 2008.

CDC’s revised Technical Instructions to Civil Surgeons for Vaccination Requirements are posted on-line at http://www.cdc.gov/ncidod/dq/civil.htm. USCIS has posted a list of “Frequently Asked Questions” about the revised vaccination requirements at: http://www.uscis.gov. For additional help with the Form I-693, call USCIS’ Customer Service Center at (800) 375-5283.

I just want to ask to those who have undergone with these, please let me know how much you paid for this.. Thank You!!

If your medical exam was before June 1, 2008 but within the last year, you do NOT need another exam and do not need the new vaccines.

Your exam was less than 1 year ago, so you do not need to do another exam.

Since your exam was before June 1, 2008, you do not need the new TB test nor the new vaccines.

Read the instructions to form I-693, Section III Frequently asked Questions, item 2. It tells you exactly what you need to do if you had an exam less than 1 year ago.

Form I-693 instructions: http://www.uscis.gov/files/form/I-693instr.pdf

The bottom of this PDF has a table that tells you if you need the TB test and/or new vaccines based on the date of your medical exam.

There is alot of conflicting information regarding vaccination requirements and the new form.

If your physical is less than one year old, all that is required is the vaccination supplemental which is Part 2 of the revised I-693.

For $20 Dr. L E Arnold of Austin, TX (Civil Surgeon) will review your vaccination forms and transcribe the information on the new I-693. If additional vaccinations are required, you will be advised accordingly. You will be asked to provide two signed copies of the first page of the I-693 and a copy of your vaccination records. His staff is friendly and helpful.

Dr. L. E. Arnold

USCIS directory has Suite number listed as 106, but they recently moved the Doctors office

Hi. I would try printing out the information from USCIS. See the link above provided by Kid Brooklyn. Print that and bring it to the doctor.

You are NOT required to be in compliance with the new requirements according to that chart because your medical was before June 1st.

AOS & getting the SSN & the I-693

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!
