attachments to forms DS230, DS156 etc.

Hi everybody,

when you didn't have enough room to answer to some questions in the forms, did you just add a white paper

mentioning maybe on the top of the sheet a title like for exemple "attachment to form DS230, question no.

23", and then you wrote your reply since in the form there was not enough space?

Do you think it is going to be ok like that as attachment, or maybe you have also to sign it and put the date or something like that? Anyone did like that?

I did I just added a typed list I didnt sign it

Quote taken from the general filing instructions:

If extra space is needed to complete any item, attach a continuation sheet, indicate the item number, and date and sign each sheet.

I did attach an extra sheet marking as you said, on my employment and dates visited, I signed and dated the docs at the bottom

We typed all of the extra information answers for each question on 1 sheet of paper. At the bottom my fiancee signed and dated the sheet. This was for her DS-230 part I from packet 3.

I put my reference number, and name, and the form number and question it was an attachment to - I also marked the form 'see attached sheet'
