Dear VJ members

i arrived to USA , California MAR 22, 2008 K-1 Visa ,

we got married April 2 and we applied for SSC April 8 , now we are filling AOS

the forms i have are

I-485

g-325a

I-864

I-765

I-131 ( i don't know if i should file it or not , because we THINK i don't need it )

i have some questions :

1 - in the form g-325 (Biographic Information) i see 5 pages 4 are similar to each other and the last one is just Instructions , do i need to fill them all , and does my wife need it too ?

2 - i have seen some ppls filling the this form DS-3025 Vaccination Documentation Worksheet , i have no idea how to do it :wacko:

any thing will be appreciated

You do not fill out the DS-3025. If you have one it was prepared by the panel physician at you visa medical. If you do not have it, then you need to take your vacciantion record to a civil surgeon and have it transcribed onto the Vaccination Supplement (sometimes called the I-693A)

g-325a is a four part identical form... if you fill out the computer version, you fill out page one and it replicates automatically.

2) You need to take foreign record to a US Civil surgeon and get vaccination record transfered to a supplement.

we couldn't find a civil surgeon in our area that would do the I-693 vaccination supplement for less than $200.

if you have the same situation once you check them in your area, search this forum for Dr. LE Arnold in Austin, Texas. For $20, you can mail them your information and they will fill the supplement out for you and mail it back.

5/8/08 - check cashed

5/10/08 - Biometrics letter received

6/10/08 - case transferred to CSC - YAY!

7/11/08 - rcvd emails that EAD card ordered and AP approved!

7/18/08 - AP rcvd

7/19/08 - EAD rcvd

7/29/08 - received email: card production ordered

8/2/08 - GREEN CARD RECEIVED IN MAIL!!!

We never had that form filled out by a civil surgeon nor did we have the form from the medical. I copied Andre's immunization record book from Jamaica and sent a copy with our AOS. We were approved without an interview.

I myself don't want to file form I-131 i really don't need it. You

would have gotten form DS-3025 from the doctor at the timeof your medical. However the information on this form needs to be transfer to the form I-693A by a civil surgeon. Google Civil Surgeon in your area call them up and see if one will do it for a small fee shouldn't cost more than $50.

I had one the other day told me it will cost $300 and i will have to do a full medical,NOT SO. I whish for you a speedy process.

09/26/07: Petition left NVC

10/02/07: Kingston Embassy received Petition

10/04/07: sent off DS-230 part 1 (sent via courier service)

10/09/07: Packet 3 received

10/09/07: Drop off DS-230 part 1 (in person)

12/03/07: Medical

12/18/07: INTERVIEW DATE!!!!!!!!!!

01/04/08: VISA IN HAND!!!!!!!!

01/11/08: Arrived in the USA

03/23/08: WEDDING DATE

09/19/08: AOS approved

09/25/08: GC in hand

That's it, Kingston. I refused to do it unless we needed it. I vowed not to do a thing till someone told me they wanted to see it. They didn't. So, I didn't.

Note many C.S. will try to get you for a full medical when only a supplement is needed, you may have to insist on just the supplement.

You are just lucky you did not get an RFE, the I-485 instructions indicate a supplement is required.

anecdotal evidence indicates that the DS-3025 is sufficient by iteself in many cases, and does not require further transcribing to the I-693a by a civil surgeon.

i called more than 13 of them just a while ago and the less one was 80$

an other one my wife knows charged her 100$

so we will go for it because i don't want to get RFE
