What do I need to bring to the interview?

Timeline

In the Notice of Action it says that I must bring:

1. A completed medical examination (Form I-693) and vaccination supplement in a sealed envelope (unless already submitted).

I had already my medical examination at the embassy (medical institute associated with the embassy) in Italy when I got my K1 visa; they also gave me 3 shots of vaccinations (that cost over $100).

I don't have any Forms that prove that because I gave the sealed envelope to the immigration officer at the Airport in Chicago when I came here to the USA.

Do I need to bring any extra Forms? I tried to call the USCIS at the number 1-800-375-5283 but it was all automated messages that didn't help.

Timeline

was your vaccination record complete before you got to the states or did you have to get more shots stadeside? if so, take the proof of it with you. if not, and your record was complete then you dont need a new medial. unless its longer than a year ago, then you need a new medical.

did you submit the I693a with your aos application? if not you have to get the ds3025 transfered to the I693a by a civil surgeon registered with uscis.

I didn't get a RFE, so I think that I don't need the I693a or anything else.

I tried to call the doctor Arnold and will call again the USCIS tomorrow morning.

I got shots in Italy for Varicella, MMR and Diphteritis, and the USCIS must have

a proof of that because they also tested my blood for HIV, etc... in Italy.

What is the ds3025 ?

the ds3025 is a vaccination sheet that the doctor in italy filled out and submitted to the us embassy. he was supposed to give you a copy of that!

i dont really know if you still need to bring the I693a for the interview, i havent had one! but the instructions say medical and vaccination supplement (I693a) if not already submitted.

maybe someone else that had an interview can help you out with that.

What is the ds3025 ?

Typically they dont send out an RFE unless they send it to California to approve without interview, in that case if you did not include the vaccination supplement with the AOS package, they will send RFE for it so that they can continue on the case.

In your case you are being interviewed so instead of RFE, they simply ask you to bring it to the AOS interview.

If you did not get your foreign vaccination record transfered to a supplement in the USA by a US Civil Surgeon you will need to get this done befor AOS interview, when you do visit the doctor, INSIST on just doing the supplement, many will try for a full medical.
