HELP! I-693A form

Hi to the board! We are putting together our AOS packet today to mail off tomorrow and I am really confused about the I-693A form that is referenced in the visajourney guides for AOS filing. My husband came here on a K-1 visa one month ago. We were married 2 weeks ago and are now trying to get his AOS done. He had the standard medical at the Embassy Doctor's office in London Jan 7th, 2008. We have his vaccination booklet from his GP that he took to the medical, the receipt for the medical, and a copy of the 2 extra vaccinations he needed at the medical. Do I just make copies of all this? The guides say that K-1 visa holders who have a medical in the last year do not need to submit theI-693 form but rather the I-693A which I cannot find on the USCIS website. am I missing something here or just having a really blond moment??? thanks for any help in advance!

the I-693 vaccination supplement can only be completed by a Civil Surgeon. bring your vaccination records to a CS so they can be transcribed to a I-693 vaccination supplement.

did you get a copy of the vaccination worksheet from the CS at your medical aboard ?

if so you can send bring that to a local CS to get transcripted to a I 693A ,

or even give Dr Arnold a call he will give you instractions on how to mail it to him to get it tranverd for 20 $

his adress you can find on top where you see medical guide on the aos forum

if you dont have the copy , you will have to take Roy and yazi advice , good luck

We used Dr. Arnold and pleased with the results.... We live in Missouri and Dr. Arnolds office is in Austin Texas, I believe....

For age 18-64 that is all you need. Some offices here are getting a bit ####### about the TB skin test as opposed to the chest xray but wait and see if you have a interview and get RFEed before you go ahead and get that, the majority of people dont get asked for that to be done.

You can try and just send in your vacciantion worksheet DS-3025 but note that you may get a RFE for the supplement to the I-693, again, some do and some dont.

Dr Arnold is great for the supplement and it is only 20 bucks and takes about a week or so with postage.

I just called Dr Arnold. The lady wanted me to send copy of documents. On form DS 3025, i only have 2 vaccination MMR and TD. and Varicell was dont by after gettign visa as per doctor suggestion. So i have 3 vaccination record( MMR, TD and Varicell) on vaccination card? So can i send copy of the vaccination card which have all 3 vaccinations?

And on the supp. form i should include my name as in card or should i inlude married name?
