Do i need to get the I-693 or I-693a by a civil surgeon?

Timeline

The day before the interview at the embassy i went through the medical examination in Italy; they gave me also 2 shots, one in each arm for Varicella, MMR and diphteritis; they also made an HIV test ( i had to pay extra money for all those shots and the test). When i came to the USA i gave the envelope to the immigration officer at the airport, i could keep only the chest x-rays. Do i need now the I-693a by a civil surgeon or not?

Although some people have just sent part of their original medical, according to the rules - yes, you need the supplement I-693A from a sivil surgeon (hope you have a copy of your medical on you).

if you have a copy of your worksheet , I would get in contact with DR Arnold , ( many people here used him including me hes amazing and only 10 bucks )

According to the USCIS you need to get a I-693a from a civil surgeon, but many of us just sent in the DS-3025 with our AOS and were approved without RFE or interview. In our case we were approved even though she was missing the TD vaccine! If you do it this way, even if they send of an RFE for the I-693a it's just a small delay and no penalty.

You probaby won't need the x-rays.

I am a worry wart so got the I-693A and also included a copy of the DS3025. Claudeth needed a tetanus shot because they didn't have it in Manila so that made me think we needed the supplemental.
