I got many vaccinations at the medical examination the day before getting the K1 visa at the Embassy in Italy.

I gave all the documentation at the POE in Chicago when i arrived to the USA.

How do i know now if i have to include the I-693a with the filings of AOS?

Is there someone that i can call and ask?

If you got your DS-3025 filled when you went to the medical exam, a certified doctor can put the records in the I-693a. I have read in this forums than many people have sent a copy of the DS-3025 and they have not had any RFE for that but anothers have got it...

Good luck
