You do not need a Medical for AOS from a K-Visa, unless you did medical for K-Visa more than a year ago.

You only need to get foreign vaccination records transfered to a supplement, by a US Civil surgeon.

Search this site for "I-693A" you will get plenty of answers.

I-485 directions:

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment of status application.

So, I would just show up at the surgeon's office and ask for a supplementary record for my foreign vaccinations?

Yes you take your foreign vaccination record to the doctor's office and get them transfered to a vaccination supplement, fee should be 30-40 for this service.

Bring a copy of the I-485 directions with you and hilight the section I pointed out. Many C.S. like to hit the unsuspecting and uneducated prospective immigrant for a full medical and chard 300+ for something that is not required of K-Visa holders.
