Approx Waiting Time for Interview After Consulate Recieves NVC Package: K1 applicants schedule interviews on their own on USTravelDocs/CGI Account. Dates are usually available 1-2 months ahead (much busied during summer, May – Aug). Immigrant Visa applicants wait 2-6 months for their interview to be scheduled.

After receiving Packet 3, you don't need to send anything back. Only prepare documents mentioned in the checklist, fill online form DS-160, schedule the medical and make your interview appointment.

Forms to Consulate? Since October 2013 doesn't require sending any documents back.

1. Passport valid for 8+ months after the interview
2. Birth certificate (original + copy + English translation if not in English or Polish)
3. Marriage certificate (if applicable; no apostille needed, certified translation if it’s not in English or Polish)
4. Divorce decrees (if applicable; copies are fine + English translation if not in English or Polish)
5. Two 5x5 inch passport style colored photographs
6. Police clearance certificate from every country applicant has resided for 6 or more moths since 16yo (English translation if not in English or Polish)
7. Court records if applicant has been convicted of any crime in any country (certified copies + English translation if not in English or Polish)
8. Military records (if applicable + for all applicants from Belarus; original + copy + English translation if not in English or Polish)
9. I-134 (a scan of the signed form is fine) + most recent tax transcript or W2 or 1099
10. DS-160 confirmation page
11. Evidence of ongoing relationship
12. Medical examination results (in a sealed envelope)
13. Appointment confirmation

The embassy sends an email once the passport leaves the post with the tracking no. Cost of Medical Exam? Warsaw: 350 PLN (for physical exam) + additional fee for chest X-ray + additional fee for blood test for syphilis + additional fee for urine test for gonorrhea, if applicable + cost of vaccinations, if needed

Warsaw accepts medical examination results fron physicians based in Wasraw, Krakow, Minsk, Moscow, and Lviv.

Estimated waiting time at the consulate on the day of interview? 2-4 hours. Immigrant Visas (+K1) seem to be processed in the end.

Tetanus and diphtheria
Measles, Mumps, and Rubella (vaccines OR antibodies tests)
Hepatitis B (Warsaw physician might accept antibodies tests, others call for vaccine)
Varicella (vaccine, antibodies, or proof of history of varicella)
Influenza (seasonal, vaccine needed if your medical is scheduled in Oct-March)
Covid-19 (WHO approved vaccine needed; for applicants from Russia – vaccine waiver D, vaccine isn’t available in Russia)
