  2. Hi there! I decided to keep my dad's last name as my middle name, however, when I had my Biometrics, the woman there told me to use my middle name when I was single because it's different here in the US. They also said that if I really want to use my dad's, then I have to go to court or I should just wait for my AOS interview so I can explain to the IO the situation.

    But when the girl inside typed my name on the computer, she just looked at my biometrics appointment letter, not the form I filled out (it's the form where the woman erased my dad's last name as my middle nameand changed it to my mom's). So on their file, my middle name is still my dad's. So, it depends. If you want it the Filipino way, then do it. Just be prepared cuz it might be an issue when you have your biometrics.

  3. You mention that the online status went from "initial review" straight to "card production", but your timeline mentions that the case was transferred to CSC. Did you call or did they mail you the information that your case was moved to CSC? Thanks

    They mailed me a letter that my case has been transferred to CSC (it also showed on their website). It has been under Initial Review until this afternoon, they sent me a text that my case has been updated. When I looked on the USCIS website, it was already under card production. And just a little bit ago, it went to Decision. Just to tell me that they already mailed me a notice that they ordered production of my Permanent Resident card.

  4. You can check your state's DMV website. I've been waiting for my EAD to get approved so I could take the written test, however, I called and asked what sort of letter or form from the USCIS they accept and they told me that I-797C is fine.

    Btw, I'm in Ohio. Showed them the Notice of Transfer to CSC, passport, marriage cert and Social Security card and I got to take the test. Got my temporary permit a few minutes after.

  5. Let her show the blue paper to the guard, it's a proof that she needed to go back to the embassy to submit some papers. If still they won't let her go in, like what Tahoma said, call the landline and just be prepared to have the Manila case number cuz they will be asking for it so they can answer her inquiries better.

    i am going to send our package tomorrow for the AOS.

    yes i have read the guide.. but this is what confuses me.

    i have already gotten my vaccination record or DS-3025 from ST.LUKES before getting my k1 visa.

    aside from the form i have received a cd for my xray from ST. LUKES

    given these information: i came here as a K1 VISA HOLDER, no kids, and just got married last month (September 2010)

    i need to ask you the following questions:

    1. do i give them the original DS 3025 or will a photocopy suffice?

    2. do i need to send them this cd too?

    3. do i need to fill out the Form I-693? coz the site says I don't need to fill out that form as long as i have gotten a medical examination within a year. follow up question: if this is so, why does it say that i have to show an evidence of the REPORT OF MEDICAL EXAMINATION AND VACCINATION RECORD... is this the DS 3025. coz my DS 3025 is just a vaccination documentation worksheet.

    4. is there another form that i have to fill out to be attached with my DS-3025?

    Look at the results on the lower left part of your DS 3025, if your vaccination history is COMPLETE, you don't have to go to the CS for the transcription, however, if it is incomplete, then you need to have your DS 3025 transcribe by a CS. You can find more threads about the I-693 under the AOS forum. When you go to the CS, you will just show the DS 3025. You only need to fill out part 1 of I-693, take it to the CS.

  7. Called the International Institute here in Toledo, OH but I was given a wrong information by the lady in-charge about AOS. She told me that I need to have another physical exam even if I had a complete medical exam last January. By the way, the nurse referred them to us because the first time we went to see the CS, the nurse told me that the CS won't transcribe my I-693 if I don't go through another physical exam and tb skin test. Called the USCIS, and went back to the hospital and the nurse that I talked to was more knowledgeable about the process than the nurse that I talked to the other day. We only paid $29 for the transcription, no physical exam or tb skin test.

  8. Before calling some CS in my area, I did a lot of reading regarding the I-693. Looked through the list of CS in the USCIS website, there are only 2 places in downtown Toledo. The rest are like 30 min. drive from my house.

    Had an appointment in St. Vincent the other day. Went to the Occupational Health Dept. The receptionist asked me to fill out some forms and papers, one of them is letting them perform a Physical Examination, but I told her I won't be signing it cuz I don't need it. Waited for a little bit then heard my name called by a nurse. Once inside, she told me that a CS won't sign my I-693 if I won't have a physical exam test and TB skin test. I showed her the page in the I-693 instructions about not having to have another physical exam anymore cuz I just had my medical last January. Still, she told me that they should need a proof that I had my medical in my country. My husband asked how much it would cost us if I would have another exam, and she said that without the DS 3025, it would be $300, however, I already have my shots, so it will only cost us $150. My husband then told her that we will just have to go back cuz I will be making some calls cuz of the conflicting information that we get. She then said that if we really want to only have the transcription, she can do it right away, but I said no. Before we left, she gave us a piece of paper and told us to call and asked them if I really need to have a physical exam or not.

    That afternoon, I called the International Institute in Toledo and asked if I need to have a physical exam, and the lady in-charge said yes. I explained to her about the instructions, and she said it doesn't matter. I was getting confused and thought of calling the USCIS just to make sure. Was able to talk to a customer service rep. after waiting in line for almost half hour, but like what I already know, it's only the transcription of my vaccination records, and if I need to have another shot, so be it, but no physical exam anymore.

    Yesterday, I called the hospital to set another appointment and told them that I talked to a person from the USCIS. Today, we went there and the receptionist (a different one) told me that I should fill out forms again using my maiden name because the name in my passport is still my maiden name. I explained to her the situation but she didn't listen, she still wanted me to use my maiden name and had me fill out another I-693 (I had mine filled out and printed at home), so I had to do what she said. I was thinking that we would go to the same nurse in my appointment the other day, but then the nurse today was a lot more knowledgeable about the process. She had me sign the I-693 that I brought and praised me for being really prepared (she said a lot of applicants would go there empty handed and didn't know what they would do). She said that I won't be needing another physical exam and tb skin test. She was really good and explained everything to us. Finally, she said that everything was fine and no need for me to have another shot because the CS told her that the MMR can be one or 2 shots (I had one back in Manila). We only had to pay $29 for the transcription.
