    I got back from Warsaw yesterday night.  I was going to respond to you after I Skyped with my wife today but that will not be for awhile.  Below is what I remember what happened and I will update this if I have anything wrong.  I will also update this to tell you when my wife's visa is available to be picked up:
     
    My wife brought copies of my AOS, this year’s tax return, last year’s tax transcript, my last two 1099-R documents, and an income verification letter, and none of these were collected or even looked at.  She brought originals and translations of her birth certificates, marriage license, change of name document, Ukrainian police report, and her Polish police report and she also brought copies of all of those documents.  They looked at all those documents but only kept the original police reports.  My wife brought two passport photos and they didn’t keep them (maybe they made copies of them but I don’t know that).  My wife also brought photos of us, copies of emails, and copies of Skype calls and none of these were looked at.  My wife was in the embassy for about 2 hours and 15 minutes and she said the interview lasted about two minutes and she was asked about 6 easy questions.  I’ve heard many people say it’s better to be over prepared rather than underprepared I would agree with that.

    I have a final update: I finally was able to Skype with my wife after after her 17 hour bus ride back to Ukraine.  My wife's interview was June 5 at 8:20 AM.  She received a message Wednesday afternoon saying her visa was ready to be picked up.  If she had been in Warsaw she could have picked it up late Wednesday afternoon.  When she  received the message she was at her Sister's house which is a two hour train ride to Warsaw.  She ended picking up her visa Friday afternoon.

    Thank you to those of you who posted your DQ, IL, and interview dates.  That information gave me a a very good idea when my wife would receive her interview letter.  After seeing your postings, I told my wife weeks ago that I think she would receive her interview letter on Friday April 21 or Monday April 24.  She received her interview letter today (April 21).  Below are all the dates that were posted by people on the last few pages of this topic.  Thanks again for sharing your information.

    My wife’s interview is June 5.  I emailed the medical office for an appointment Friday.  They emailed me back and said the following:
     
    At the moment we can make an appointment for you at the end of May as our schedule for June is not open yet. Would May 31st at 10 AM work for you?  The doctor's appointment consists of a physical and immunization review. If any required vaccinations are missing, they will be administered during the doctor's appointment. After the appointment the doctor refers the applicant to the facility at 9 Ludwika Warynskiego street in Warsaw (no appointment necessary) for a chest X-ray and a syphilis blood test (if age appropriate). The medical documents are submitted to the Embassy via an electronic database within 1-2 days, if none of the lengthy tests are applicable (please see price chart).
     
    From their response it seems like they would have made the appointment even closer to my  wife’s interview date if they were already making appointments in June.  My wife wanted to go earlier so we made an appointment the 2nd week in May.

    Hey, you go there through register an appointment, choose "Appointment made by NVC", then visa type and etc. , I chose to pick up my passport at Weirtnicza and in the end it gives you a page with the bar code that u need to print.

    My wife lives in Ukraine and is a Polish citizen so we are staying with the Warsaw Embassy.
