When you go to submit it, before you actually submit, the fees option will pop up. This is where you pay the fees. You can use debit, savings or credit card to pay these fees. We used credit card... might as well get some points!

No. Use your current name. The marriage certificate is the legal name change document to connect the two names.

Make sure you have a new passport in your new name before the NVC step (DS-260, etc.). The embassy can only issue visa with the same name as the passport.

Not a problem, file with your new married name and make sure to update any IDs, accounts, your passport (especially if you are planning on visiting or going out of the country during the process) etc to reflect your new name
