Also, does anyone understand the reasoning why the medical people need to see a police certificate? I can't make sense of it. And do they hang onto it and pass it on to the embassy or will we get it back?

My police certificate was printed the end of October last year. I used this at my medical two weeks ago. They take a copy and give it you back. 
In the email I received from Visa medicals it states a police report within the last 12 months as does the embassy.

No I don’t think so. You pick a collection point when you register your appointment that’s nearest to where you live to collect your visa etc. After this you can then upgrade to premium delivery which delivers direct to your address instead.

Is that the premium delivery?

You register your appointment via the link, you can upgrade to premium delivery and pay online - it’s £19. 

I applied for the ESTA while my I-130 was pending and travelled to the US without any problems ( I am also from the UK).
