We did 1/2 page for each month of chat logs for CR-1. The primary purpose was to show the communication has been continuous.

Try to chose some from many different months to show how long you have been in communication. Also content may be important. Meaning showing meaningful things that you and your fiancé have talked about like marriage, kids, work etc

Each country is different and would be in the country's currency. Trinidad has free clinics that vaccinate so I only had to pay for the varicella (chicken pox) shot that was available only from a private medical facility. 

I needed TDap, MMR, and flu. Those are the usuals that most people need. Most others are not age appropriate. I paid $45 for the flu shot at Walgreens. I paid $8 each for the TDap and MMR at my counties public health clinic. They are close to $100 each at Walgreens when I checked. The nice thing about the public health clinic is that they transferred all of my vaccine record from Mexico into the official Texas database, so now I have a complete record from in the US.

Adding to what others have correctly advised, once you know which vaccines you will need, if any, you may choose to get them wherever you wish. You can usually find them cheaper at clinics, or near free from your county's public health center. I have even heard of a few CS who include vaccines in their costs for the exam. Not many, but some. They are usually the most expensive option, at least that was the case when I was calling around to find out prices.

Once the medical facility looks at your records and immunization history, they can tell what updated vaccines you would need. When I did mine, out of 8 listed vaccines, I only had to get updated for 3.
