Put it down especially if it's on the birth certificate. I had to put my biological father down even though I have never had a relationship with him. If you have the information for him, put it down.

"Unknown" if you truly don't know the answers to the questions being asked about the father.
If you DO know, or can find out (ie. ask the mother) then fill in what you know.
(For K1 purposes, they do not care whether the father has been in the beneficiary's life/acted as a father should---they only care about his biological data)
