    This is what the embassy just wrote me:

    "Your fiancé is not required to have a DS-3025 completed by the doctor; he will complete this form

    when he applies for adjustment of status in the United States, after you are married."

    I find this very interestng. I hope that he can write the vaccinations on DS-3036 - Medical History or on the DS-3025 Vaccination Documentation even though it isn't needed so that he has a record to show the civil surgeon in the US.

    My fiance doesn't personally have any record of his vaccinations he lost it about 2 years ago. But he will go to the hospital and see if he can get a copy.

  2. For a K-1:

    I am aware that once the beneficiary comes to the US and marries that vaccinations will need to be done as one of the steps. Additionally, I know that the person can choose to have the required vaccinations in their own country or the US. But my question is about prior to US arrival, when the petitioner/my fiance goes for his medical he needs to do the:

    DS-2053 medical Exam

    DS-3024 chest x-ray

    DS-3026 medical history and physical exam. worksheet

    But what about the DS-3025 Vaccination Domumentation Worksheet? Does the doctor need to do that even though vaccinations for my fiance are not needed until after arrival in the US. I thought that I had read that the vaccination sheet needs to be in the packet at POE. I ask as the Embassy tip sheet included in Packet 4 doesn't state that it needs to be done. But if it isn't done then how will the US doctor know what vaccinations my fiance had in his own country?

  3. I am the USC filing a K-1 for my fiance but the embassy is asking that both he and I provide our Parents birth and marriage certificates. I am age 40 and they are requesting info on my parents who are in thier 60's. And the information they want I have no access to. The only way would be if I go to my parents and ask them to order these forms for me. Which I will not do and I know if asked they would not provide them. I can not understand how I can be asked to provide documents of which I am not legally able to obtain myself.

  4. If you click on 'guyana' in the left margin, under your name, you'll go to a list of other users of your Consulate. Maybe one of them can answer your question--it is *very* Consulate-specific.

    Thanks I did that - there is only one person who was appropriate to contact. At there interview 1 1/2 years ago it was not needed.

    What I do not understand is how can the US gov't expect a USC to provide information that is not accessible to the USC!! It is truely permittible for them to ask that I provide documents that only my parents have access to?

  5. I called the embassy - the woman said I need it. I e-mailed the embassy and I am awaiting a response. I would love to know if anyone out there needed to present it, and didn't and still got the visa approved. I just called to Vitalcheck in NY which handles the birth, marriage and death certificates for the state of NY and I am unable to request the documents personally as my parents are alive.

  6. I just got the correct Visa Tip Sheet from the Embassy for the K-1 visa.

    For the Applcant and the Petetioner it states:

    Please place your documents in the following order:

    Civil documents

    1. Birth certificate

    2. Marriage certificate(s) (if any)

    3. Death certificate of spouse (if any)

    4. Divorce decree(s) (if any)

    5. Parents birth and marriage certificates

    Is this true that they need my parents birth and marriage certificates????? I do not have access to this information! Besides, I'm age 40, and my parents were divorced over 25 years ago. This is info I am not able to present in xerox or original form.

  7. Does the packet not include the information regarding the $100 visa fee and the option to pay in advance at Citizen Bank? If they require fingerprinting (can't imagine why unless you are filing a waiver) they would normally also provide information regarding the cost and the options for payment at the consulate in the packet.

    they sent us the wrong "Immigrant Visa Tip Sheet" it probably should of been non immigrant tip sheet. Regardless the one they sent said you pay at the embassy, and I know for the K-1 you also pay at the embassy or it said to NVC. But it didn't state if my fiance could pay by money order or certified check. He difinetely doesn't need to go to Citizen's Bank

  8. Hello folks,

    I'm very frustrated at the moment. My fiance finally received his packet 4 yesterday after it taking about 2 weeks to arrive and we believe that he received the incorrect one as there was no DS-156, 156k and 157 and the fee at the embassy is $380 and they want the I-184, plus it requested the petetioners original birth certificate. So I wrote the embassy this morning about the above and I'm awaiting a response.

    1-So is the packet supposed to be for NON IMMIGRANT visa or IMMIGRANT visa? It appears that we got the immigrant one.

    2-Also I heard that the fee should be $100 plus a fee for fingerprinting? correct? Can I pay it by certified check or money order?

    3-For the K-1 has anyone needed the petitioners original birth certificate?

    Update: The embassy just called him and told him that indeed they did mail an incorrect packet and that he can pick it up in person tommorrow. In the mean time I received this e "Please be informed that the correct immigrant visa appointment package is available for pick up from this office; we have notified your fiancé by telephone. We regret the inconvenience caused."

    They haven't as yet responded to the questions I just posted here.

  9. I suggest look at the website more thougherly. For example for the Guyana embassy - when I go to the visa page - it gives direct #. Or call to a different dept in Babgkok embassy and state that you are trying to reach such and such dept but you are not able to get through can they connect you or forward a message. Also try e-mailing the other embassy in Thailand and express that you haven't been able to contact Bangkok do they have a # or email that you do not?

    If the question is asking for past income as reported on a tax return, yes. If the question is asking for current income, no. Unless, perhaps, one is self-employed and a past tax return is the only way that one's income can be documented.

    The words on the I-134 read "I derive an annual income of" Since they only wanted my 2005 1040 and I did not feel that it reflected a typical 1040 I decided to write both the W-2 amount and gross income and marked them as such. I included my 1040, W-2, and letter from my bank which listed the monies deposited from my place of employment and I added an attachment expressing the large deposit as sale of real estate (which shows up on the 1040 as capital gains).

  12. So I just called to Visa Inquire 202-663-1225 they said that it is an immigrant form not a state depart form and to call them at 1-800-375-5283 - I'm on hold for 20-25 min ugh - but I need to send this out today! Well they were no clear help - do what you think!

    I would think that the I-864 and the I-134 would use the same info regarding income.

    The line for gross (total) income on IRS Forms 1040 (line 22 in 1996) and 1040A (line 14 in 1996) is used to determine income. The line for adjusted gross income is used for persons filing IRS Form 1040 EZ (line 4 in 1996).

    I'm filling out the I-134 and also am not sure what to put - my accountant did a 1040. I do not see the words "gross income" on the 1040 form, but I do see the words "adjusted gross income" This year due to sale of real estate I have very different amounts of money on the 1040 form. Line 7 on the 1040 form reads "wages, salaries and tips (W-2)" for me which was 48K and line 37 "Adjusted gross income" reads 66K so what # do I put next to "I derive an annual income of" No where in the directions does it give any directions on what to write.

    I also want to know what to put down. For myself last year I sold a property that I had owned so My W-2 showed 48K for wages, salaries and tips but line #37 on my 1040 shows 66K due to the sale of real estate so do I write 48K or 66K????

    Additionally, they ask for deposit in savings bank, personal property, stocks & bonds

    1) Is deposit in bank account solely savings or is it savings plus CD's at the same bank

    2) is real property my car's value minus the loan payment?

    3)Is stocks and bonds my IRA, and other retirement? does it include the CD's (certificate of Deposits) here rather then in savings?

    Country - Guyana

    State/Province

    The passport from guyana doesn't list City or State/Province. It only has an oval stamp that reads Passport Office, Republic of Guyana. Do I write "None", "N/A" or leave it blank next to the words City and State/Province

    #18 Spouse's Full Name (Even if divorced or separated. Include maiden name.)

    This is confusing, does he write his ex-wifes maiden name?

    #19. Spouse's DOB (dd-month-yyyy)

    I guess this will depend on # 18 - if he writes his ex-wife name he would place her birthday here.

    #20. Name and Address of Present Employer or School

    #21. Present Occupation. (If retired, write "retired". If student, write "student".)

    He stopped working 1 month ago - all his former applications and forms showed his then current employment. So do I write "None" or Formerly ABC Shipping Company for # 20 and "None" or Seaman/Stopped work 1 month ago for #21

    #28. Who Will Pay For Your Trip?

    Does it look bad if he states his fiancee?

    I his fiancee and filling this out for him on the computer, I want to fax it too him, will it be accepted with a faxed signature rather then an original signature by me. It would then have his original sinature as the applicant?

    As of yet we haven't received packet 4 which I gather will contain the appointment letter within based upon your response - Can a person pay the fee to Citizen Bank without the appointment letter? I ask as my fiance will be in Georgtown this week. Is the actual appointment given after paying the fee or will it arrive in Packet 4?

    Additionally, on a gov't web site I saw that the fee for the DS-230 form is $335. So far my fiance only filled out Part 1 and sent it back to the Embassy. When does he do part II and will he be required to pay the fee of $335?

  18. At the time that the beneficiary attends the interview at the embassy are any funds needed? If so how much and can it be by certified check or money order. As if there is a fee I will forward it with the Affidavit of Support when I DHL the papers down to him. In the same regard is any money need to pick up the visa or upon entrance through customs into the US.
