Haven't talked to her yet to get the final word but I think she was approved.. I've been checking the status and it just changed to issued... Is that a good sign?

It is totally unnecessary to write N/A 38 times. If you don't need the entire space to answer a question, so be it.
As for the prior spouse question, write none once and be done with it.
I understand the very nature of this process lends itself to paranoia, overthinking, and second guessing (I was guilty of all 3). However, "box" in this case refers to a whole section, not the lines to provide answers.

I would imagine you'd send them to the Service Center, but I'm not a hundred percent so maybe wait for someone else to chime in. It might be easier as others have said, if you scanned them and printed them as you could fit 2 to 3 on each page. That's you choice though, you can send real photos in a baggy also.

Green Card holders can petition for their spouses! It just takes a lot longer, since those visas are limited, if I'm not mistaken.
