  1. also i would like to add that this what i used to get my american passport and had no problem and also what i sent with my i-130 aslo this is what i found on the web n it says nothing bout birthdates of parents..

    • Long form - legal size or two page form with details of the person, their parents', place of birth, certification by the parents, signature and stamp of the issuing agency or department. Recent forms are in English and French.
    • Short form or card - provides name, birth date, place of birth, date of registration, date of issue, registration number, certificate number, signature of registrar general
    • The passport office tells everyone to get a long form because then there is no doubt that it will be accepted. Some states have short forms that comply with passport requirements, and therefore they are acceptable. Basically, as long as the birth certificate has your parents' names, it will be accepted.
    • LONG FORM birth certificate will show the name of the child and the names of both parents. If only one parent was declared, then it will show the child's name & the mother's name.

A SHORT FORM birth certificate will show ONLY the child's name.

For an adult (age 16 or over) a short form should be fine. However, for someone under the age of 16, parental consent is needed. There is no way to know who should be giving parental consent without a long form birth certificate showing parent(s) names.

  3. Hi friends was wondering if anyone knows exactly what address my husbands packet will go to as i am currently here in egypt with him but i will be going home soon due to finding out i am pregnant. Our i-130 has his home address on it where he lives with his family but on the second page it asks for a address where you last have lived together so i put that address as well to make sure they know we have lived together.. just wanting to make sure my husband is prepared for everything and if i need to contact nvc before i leave. Thanks any advice is greatly appreciated :)
